At Cedarview Animal Hospital we are always happy to welcome new pets and their families to our hospital! In order to book an appointment for your dog or cat, please complete the new client form below and upload any medical records you may have from your previous veterinarian. You can also give us a call at 613-825-5001 or request an appointment through the widget on the bottom right hand side of our website.
